Effective January 3, 2022, all CPLC Employees and all Service Providers, are required to be Fully Vaccinated against COVID-19. Newly hired employees, who are not Fully Vaccinated at the time of hire, are expected to begin a vaccination series immediately upon hire. CPLC recognizes medical and religious exemptions to the COVID-19 vaccination requirement, in accordance with applicable law and upon approval by CPLC. However, any person approved for an exemption, without exception, is subject to masking, weekly COVID-19 testing at their expense, and other requirements established by CPLC from time to time.PRIMARY DUTIES AND ESSENTIAL FUNCTIONSThe following statements are essential functions of this position and not intended to be all-inclusive; rather, they are intended to describe the general nature and level of work to be performed. CPLC reserves the right to revise or change the job duties and responsibilities as the need arises, based on business need, and this Job Description may be updated accordingly. Responsible for implementation of the CPLC Programs in accordance with grant guidelines. Provide case management services in the specified activities for eligible participants. Conduct intake to determine eligibility for all applicants by completing family profiles and obtaining necessary documentation per agency and program requirements/guidelines.

  • Work closely with support staff to assure all aspects of the individuals needs are being addressed.
  • Perform further needs assessments and counseling service to determine the individual's need(s).
  • Develop a comprehensive service plan based on individual and family needs and goals.
  • Coordinate support linkages (including transportation and childcare) to remove barriers to employment.
  • Develop and maintain community resource information by collaborating with various organizations, i.e. attending interagency meetings, etc.
  • Communicate with various service providers to refer clients to other available resources within CPLC and the community regarding their household's need.
  • Act as advocate for the client at the point of entry in the service system.
  • Monitor service delivery and follow-up on referrals to other agencies.
  • Provide program services per agency and program procedures that include: Completing all forms that are associated with the program application, gathering information and documentation to determine agency's ability to financially assist the participant.
  • Coordinating service delivery with various vendors.
  • Developing and coordinating training activities and support services.
  • Determining participant selection/placement (based on eligibility requirements per agency and vendor), registration and preparation of documents for internal/external approval.
  • Conducting site visits at training vendor locations to monitor participant's progress as well as collect the time and attendance records while the participant is enrolled in training activity.
  • Maintaining contact with the participants to determine the effectiveness of the training and services as well as to inquire about absences or any other issues that may impact the completion of training and employment opportunity.
  • Providing on-going individualized employment counseling and training to enhance the participant's employability skills with the Job Developers and Career Navigators.
  • Referring participants to appropriate job opportunities for employment placement, follow-up for placement and advancement data at intervals set by the work statement.
  • Prepare and submit purchase requisitions with support documentation (quotes, registration form, etc.) for all client services purchases. Prepare documentation (vouchers, etc.) for financial assistance to be redeemed by vendors. Reconcile purchase logs on a weekly basis. Assist with project budgets and planning activities as necessary.
  • Ensure accurate and timely data entry relevant to records in approved computer system software such as CAP 60, CPFR, and NMCOS.
  • Responsible for client file creation to include required documents. Maintain complete and accurate records on program and participants' activities per agency procedures/work statement; provide the Workforce Development Director with appropriate statistics for the program, i.e. weekly, monthly, quarterly and annual reports.
  • Responsible for other duties as deemed necessary by the Workforce Development Director including performing all phases of the agency's assistance program.
  • Track participant activities according to service plan.
  • Conduct required follow-up activities for exited participants as per policy.
  • Responsible for other duties as deemed necessary by the Workforce Development Director or Vice President, including performing all phases of the agency's assistance program. MINIMUM QUALIFICATIONS AND COMPETENCIESEducation /Background:Associate's Degree from an accredited two-year junior college in Family Studies and two (2) years of progressive experience in all aspects of either employment and/or training programs, social services and/or with the low income population. Any combination of education from an accredited college or university and/or direct experience in this occupation totaling four (4) years may substitute for the required education and experience.
