The Community Resource Coordinator provides resources and referrals for individuals age 18 or over with disabilities, or individuals age 60 years or older that need assistance in accessing public benefits for which they may be eligible. The Community Resource Coordinator will assist in the enrollment process for long term care services, which includes assisting disabled individuals, seniors, and family caregivers with appropriate services. The Community Resource Coordinator provides Medicare counseling and coordinates La Plata County's volunteer Medicare counselors.

Essential functions may include any of the following tasks, knowledge, skills and other characteristics. Conducts interviews with clients, assesses needs and provides Medicare Counseling, clarifies information provided as needed, and assists clients in completing necessary forms.
Provides information and resource referrals to community members and agencies.
Informs individuals of available resources to assist them and address their questions; and refers individuals to other community resources as appropriate.
Coordinates La Plata County's volunteer Medicare counselors and provides Medicare counseling
Conducts ongoing case management to continue to provide additional referrals and assistance as needed.
Raises awareness in the community regarding long-term care support services and options.
Works with families/individuals with all income levels and all types of disabilities
Collaborates with multiple stakeholders throughout the community to provide information and education regarding available community resources.
Enters and retrieves information from computer systems; reviews and evaluates computer generated data. Completes extensive data entry and documentation.

Performs other related duties as assigned.
Regular and predictable attendance is required.

Education and Experience:
Bachelor's degree in Arts, Behavior Science preferred
Minimum 2-3 years work experience in the field of social work or public services,
Any combination of education, training and experience which provides the required knowledge, skills, and abilities to perform the essential functions of the job.

Licenses and Certifications:

Certified Information and Referral Specialist for Aging (CIRS-A) desired

Current Drivers License.


Knowledge of:
Recordkeeping, report preparation, filing methods and records management techniques.

Correct English usage, including spelling, grammar, punctuation, and vocabulary.

Basic knowledge of computer database management.

Skill in:
Interacting with others through a person-centered philosophy.
Using tact, discretion, initiative and independent judgment within established guidelines.

Preparing clear and concise reports, correspondence and other written materials.

Organizing work, setting priorities, meeting critical deadlines, and following up on assignments with a minimum of direction.

Applying logical thinking to solve problems or accomplish tasks; to understand, interpret and communicate complicated policies, procedures and protocols.

Using a computer terminal to accurately and rapidly enter and retrieve data and information.

Using mathematics.

Communicating clearly and effectively, both orally and in writing.

Ability to:

Ability to respond to each client with empathy and respect.

Ability to establish and maintain effective working relationships with a variety of individuals.

Ability to define problems, collect data, establish facts and draw valid conclusions.

Working Conditions and Physical Demands:
While performing the essential functions of this job the employee is frequently required to sit, use hands to finger, handle, or feel, reach with hands and arms, and speak and hear. While performing the essential functions of this job the employee is occasionally required to stand, walk, and lift and/or move up to 10 pounds. Must be willing to participate in a minimum of 8 hours of annual required training.

$20/hr hr FT w/ medical, dental, vision, retirement & paid parental leave benefits.
