Job Description:

Position Summary:Specialized medically-complex care of adult cystic fibrosis patients. The cystic fibrosis APP has afive-fold responsibility to patients and the staff that are involved in the care of those patients. These areasinclude advocacy, clinical management, advice and support, education, research, and management. Clinicand inpatient duties occur Monday through Friday 0800-1700.Function in a specialized area of advance practice in the role of practitioner, mentor, consultant, educator, researcher and manager. Establish goals and priorities in accordance with patient and staff needs and organizational objectives. Ensure adherence to Hospitals and departmental policies and procedures. Patient care assignment may include neonate, pediatric, adolescent, adult and geriatric age groups.Accountability:PCMH/PCSP - Adhere to and promote the core expectations of the Patient Centered Medical Home or Patient Centered Specialty Practice as applicablePATIENT CARE - Assess, diagnose and develop treatment plans; implement treatment, follow-up and evaluate patient within the scope of practice protocols as outlined by state statutePATIENT CARE - Perform patient history and physical exams as needed for procedures or admissions; document findings noting pertinent normal and abnormal findingsPATIENT CARE - Perform invasive and non-invasive diagnostic and therapeutic proceduresPATIENT CARE - Evaluate and revise intervention care/treatment plan with patient and family as necessary to achieve improved healthDOCUMENTATION - Provide accurate documentation of patient care, document orders, directives, procedures and patient responses in the medical record and communicate them to other members of the health care team; maintain accurate, legible and confidential recordsROLE MODEL - Act as a role model through conduct, appearance, communication, mutual respect, leadership, ethical decision-making, critical thinking, and problem solving skillsPATIENT CARE - Provide education for patients and/or families about preventative care, medical issues and use of the prescribed medical treatmentPATIENT CARE - Work as a team member with medical/nursing/counseling staff to provide comprehensive care and promote continuity of careEDUCATION - Guide, teach, support and serve as a clinical resource nursing and medical staffRESEARCH - Support research by conducting or participating in research studies; incorporate findings in clinical practicePATIENT SAFETY 1 - Follow patient safety-related policies, procedures and protocolsPATIENT SAFETY 2 - Demonstrate proactive approach to patient safety by seeking opportunities to improve patient safety through questioning of current policies and processesPATIENT SAFETY 3 - Identify and report/correct environmental conditions and/or situations that may put a patient at undue riskPATIENT SAFETY 4 - Report potential or actual patient safety concerns, medical errors and/or near misses in a timely mannerPATIENT SAFETY 5 - Encourage patients to actively participate in their own care by asking questions and reporting treatment or situations that they don't understand or may "not seem right"DEVELOPMENT - Enhance professional growth and development through participation in educational programs, reading current literature, attending in-services, meetings and workshopsEducation Requirements:Advanced Practice Nursing or Physician Assistant Accredited Program GraduateExperience Requirements:No minimum experience requiredLicensure/Certification Requirements:Advanced Practice Registered Nurse Licensed by the New Mexico Board of Nursing or Physician Assistant Licensed by the New Mexico Board of Medical Examiners and Certified by the National Commission on Certification of Physician AssistantsCPR Certification for Healthcare/BLS Providers or for Professional Rescuers or must obtain within 30 calendar days of date of positionPrescriptive Authority from the State of New MexicoFederal DEA CertificationACLS or PALS (as applicable) within 30 days of hirePediatric Nursing Certification as applicableExperience Requirements - Preferred:2 to 5 years experience as a practitionerBilingual English/SpanishBilingual English/Keres, Tewa, Tiwa, Towa, Zuni, or NavajoPhysical Demands Requirements:Medium Work: Exerting 20 to 50 pounds of force occasionally, and/or 10 to 25 pounds of force frequently, and/or greater than negligible up to 10 pounds of force constantly to move objects. Physical Demand requirements are in excess of those for Light Work.Testing Requirements:Tuberculin Skin Test required annuallyWorking Conditions Requirements:Sig Hazard: Chemicals, bio hazardous materials requiring extensive safety precautions and may require protective equipmentSubject to UNM credentialing; must maintain credentialsDepartment: Inpt-APP - Int Med Specialty
