Southwest Region Looking to make a measurable impact in a role that
blends leadership, strategy, and patient-centered care? Join a
hospital with deep community roots that’s seeking a hands-on
Director of Care Management to help strengthen culture, drive
performance, and lead change. What You'll Do Leading and evolving a
compliant, high-performing Care Management program aligned with
regulatory standards. Coaching and mentoring a tenured team
(including 3 Case Managers, 1 PRN, and a Clerk) with clarity,
accountability, and professional development in mind. Serving as a
trusted liaison across departments and with community partners to
support safe transitions of care. Participating in LOS meetings and
Patient Care Conferences to ensure efficient, patient-centered
outcomes. Reinforcing standards and practices that support
regulatory compliance and consistent performance. Requirements: RN
and Bachelor’s required; Master’s preferred 3 years of leadership
in Case/Care Management with Utilization Review experience
(hospital experience required) Case Management Certification
