for the Northern New Mexico differential of $4.00 per hour. (This
differential may be adjusted or discontinued based on business
requirements.) Qualifications: - Associate Degree in Nursing -
Registered Nurse Licensure - Compact/Multi - Minimum one year in
direct patient care setting or home health care. - Valid Driver's
License - BLS required at time of hire Responsibilities: - Assess
patient needs, including physical, mental, financial, and spiritual
aspects - Provide skilled nursing care in alignment with
physician/APC care plans - Coordinate patient care through
collaboration with caregivers, multidisciplinary teams, and
physicians - Adjust interventions as needed based on patient
responses and updated orders - Evaluate effectiveness of
interventions at each visit - Develop and implement hospice care
plans while ensuring compliance with regulations - Maintain
productivity standards for patient visits and caseload management -
Communicate visit schedules and any changes to patients in a timely
manner - Guide newly hired nurses and students, serving as a mentor
and resource - Ensure documentation is accurate, timely, and
compliant with agency policies - Delegate appropriate care tasks to
LPNs and CNAs - Participate in quality improvement initiatives and
performance improvement projects - Take on leadership
responsibilities as required by the department - Provide holiday,
weekend, and evening coverage as needed - Perform additional
functions as required for patient care and agency operations
