Overview:Provides high quality professional nursing services in
an Outpatient Clinic.Type of Opportunity: Full TimeFTE: 1.0Exempt:

program.Qualifications:Credentials:Registered Nurse State of New
Mexico or Compact State Nursing License required. BLS certification
within 90 days of hire.Education: Associate Degree in
NursingResponsibilities:Assesses the identified patient population
based on symptoms and medical history. Develops a plan of care
based on preventative care needs, using standard based practice
guidelines. Collaborates with patients to update problem list,
current medication list and coordinates visit plan. Evaluates
nursing care, seeks to improve process and is through and accurate
in completing work. Communicates with the provider team and other
community health related persons in order to coordinate care plan.
Supports the Care Team Associates as a member of the Primary Care
