We are looking for a skilled Nurse Manager who can effectively lead our hospice clinicians to ensure the delivery of high-quality compassionate patient care.Come join our growing hospice team based in Santa Fe, NM, where we are dedicated to the health and well-being of our Northern New Mexico community.The Manager of Patient Care assists the Executive Director or Program Director in planning, directing and coordination of the financial and personnel activities of the Home Care, Clinical Services Department. Management and supervision of a team of professional and paraprofessional employees accountable for providing competent care and achieving optimal patient outcomes. Ensures adherence to practice standards, appropriate utilization of services, and compliance with federal and state regulations as well as agency policies. Provides case management oversight for a geographic team of patients/families to include consultation on clinical issues, medical management, payer identification and requirements, scheduling to ensure skills/needs match, clinical competence, team coordination, and communication with the physician.
