understand how to engage your staff. We are looking for a skilled
Nurse Manager who can effectively lead our hospice clinicians to
ensure the delivery of high-quality compassionate patient care.Come

community.The Manager of Patient Care assists the Executive
Director or Program Director in planning, directing and
coordination of the financial and personnel activities of the Home
Care, Clinical Services Department. Management and supervision of a
team of professional and paraprofessional employees accountable for
providing competent care and achieving optimal patient outcomes.
Ensures adherence to practice standards, appropriate utilization of
services, and compliance with federal and state regulations as well
as agency policies. Provides case management oversight for a
geographic team of patients/families to include consultation on
clinical issues, medical management, payer identification and
requirements, scheduling to ensure skills/needs match, clinical
competence, team coordination, and communication with the
physician. Utilizes business knowledge and agency objectives to
proactively identify staffing requirements and initiate the hiring

YesWork Schedule: DaysQualifications:*Bachelors Degree in Nursing,
Physical Therapy or in a healthcare related field, 6 years of
additional experience can be substituted in lieu of degree.*3 years
clinical experience in direct patient care, with at least 1 year in
delivering healthcare in the home environment.*3 years experience
in supervision and/or management.*New Mexico RN or PT
License.*Strongly prefer knowledge of clinical case
management.*Skilled in Microsoft Office applications.*Knowledge of
federal and state regulations regarding home care delivery and
payers.*Ability to analyze, interpret and apply data to the daily
care delivery process.*Demonstrate knowledge of the principles of
growth and development over the life span of patients
served.Credentials: Registered Nurse State of New Mexico or Compact
State Nursing License. BLS certification within 90 days of
