staff and their immediate families Summary:The Case Manager
Supervisor will report to the Chief Prevention and Recovery Officer
and oversees Case Management across SFRC's services in Santa Fe.
This position is responsible for leading and developing SFRC's Case
Managers, setting best practices and informing standards of care,
facilitating team meetings, planning and providing training for new
hires and other team members as needed, and interviewing new Case
Manager candidates. The Case Manager Supervisor works closely with
SFRC's Case Manager Supervisor in Gallup and the leadership of
Santa Fe's women's and children's program to collaboratively manage
and develop Case Management best practices and systems across the
organization. The Case Manager Supervisor adheres to all the
responsibilities of the Case Manager position and works as part of
the interdisciplinary team to facilitate and coordinate clients'
continuum of care activities, ensuring optimum utilization of
community resources, service delivery, and compliance with external
agencies and referral sources requirements. The Case Manager
Supervisor facilitates and prepares the clients for success in
their next phase of treatment and when returning to their community
and facilitates wraparound case management consistent with the
CCBHC Criteria. Duties and Responsibilities of the Case Manager
Supervisor: (Included but not limited to) You'll report to the
Chief Prevention and Recovery Officer and:Implement and oversee
targeted case management services providing intensive case
management addressing SDOH factors for individuals served,
consistent with CCSS and CCBHC criteria.Assess the SDOH needs of
CCBHC clients at admission and throughout services to determine
collaborative goals in a person-centered care environment. Ensure
the needs of individuals served are addressed throughout
treatment.Assures clients have access to resources throughout
treatment and upon discharge from services, especially during
critical transitions such as from hospitalization or correctional
settings. Serve as liaison with community services to assist the
client with identified goals and needs and provide services in
coordination with DCO's consistent with the CCBHC model. Liaison
with family, friends, partners, and outside resources to create a
supportive environment and ensure continuity of care. Educate the
participants on behavioral health, promoting healthy resources, and
daily skills training to provide the highest quality
person-centered care. Work closely with members of the
interdisciplinary care team to address daily concerns, intervening
if crisis intervention is required, and offering additional
directives for care to provide the highest quality person-centered
care. Assist participants in developing an aftercare plan so that
they will feel confident upon exiting the program that they have
the resources required to facilitate the continuity of care. Uphold
participant confidentiality in accordance with company policies and
procedures and local, state, and federal laws and regulations.
Maintain case files and other required documentation according to
agency policies and train Case Management staff to adhere to those
standards. Regularly identify and maintain relationships with new
employment, housing, medical, and other community resources.
Schedule personal business and legal appointments and general case
management for clients Ensure all documentation is completed in a
timely manner Administrative Supervision of Staff (approving
timecards/time off requests, progressive discipline, scheduling,
performance appraisal, and working with HR to recruit, hire, and
train) Maintain reports of services and deliverables related to
multiple grant initiatives fulfilling contracts including but not
limited to local, state, and federal funding.Requirements: Minimum
Qualifications:Skills/Knowledge/AbilitiesExcellent written and
verbal communication skills with the ability to effectively
communicate with diverse populations Cultural and diversity
understanding and awareness Interpersonal skills that promote and
encourage teamwork Advanced knowledge of HIPAA and confidentiality
Advanced knowledge of behaviors and emotions related to addiction
Advanced knowledge of community and referral resources for clients'
diverse needs Skills in documenting effective case management notes
Competency in Case Management and related modalities, such as CCSS,
Recovery Capital, and SDOHProficient use of standard office
equipment and systems: phone, copier, the computer including
Microsoft Office Word and Outlook tools. Mastery of E.H.R.
systemAbility to coordinate volunteer and vocational activities for
participants Critical thinking/problem solving Ability to manage a
variety of tasks and organize workload Attention to detail
Proficient in data and reporting for grant-related
deliverablesMaintain effective professional relationships with all
levels within the organization Familiarity with community resources
including mental health, social, financial, educational, housing,
employment, childcare, and other support that may be required.
Ability to build rapport with clients Education and Experience:High
School/GED requiredAddiction treatment counseling: 1 year
(Preferred) Case Management certification (Required) - ACM
(Accredited Case Manager), CCM (Certified Case Manager, RN-BC), or
CPSW (Certified Peer Support Worker) Current CPR/First Aid
Certification (or willingness to become certified) Unrestricted NM
